


Today, we pause to remember a day that forever changed our nation. On September 11, 2001, nearly 3,000 lives were lost in a tragedy that shook the world—but in the midst of heartbreak, we also witnessed extraordinary courage, unity, and compassion.
At Home Care Plus, we honor the memory of those we lost, the bravery of first responders, and the resilience of families who continue to carry the weight of that day. We also recognize the countless individuals who turned their grief into action—choosing to serve, to give, and to care for others in the spirit of healing.
That same spirit lives on in the work we do every day.
As a non-medical home care agency, our mission is rooted in service. We care for the most vulnerable members of our community with dignity, patience, and love. Whether it’s helping a senior feel safe at home, offering companionship to someone who feels alone, or supporting a family through a difficult time—our caregivers are everyday heroes in their own right.
On this National Day of Service and Remembrance, we recommit ourselves to that mission. We believe that every act of kindness, no matter how small, is a powerful tribute to those we lost. Service is not just what we do—it’s who we are.

We provide a comprehensive ongoing needs assessment by a Registered Nurse identifying an individual’s and his/her support system’s strengths, weaknesses, limitations, objectives and service needs. With the focus on health, psychosocial and environmental factors we develop an individualized plan of care based on the needs assessed. The plan of care is tailored to fit changing needs and thoroughly reviewed with the client, family members and/or responsible parties.
