Home Care Plus is an in-home elder care and support service whose goal is to help elderly client’s live life as fully and safely as possible in the comfort of their own homes. We offer a wide range of services to meet a variety of needs. Our mission is to provide quality supportive services and care coordination to our clients to assist them in leading dignified and independent lifestyles in the comfort and safety of their own homes.
Responsibilities:
-Provide compassionate care and assistance to individuals in need
-Assist with activities of daily living, such as bathing, dressing, and grooming
-Medication Reminders
-Provide Alzheimer’s and Dementia care
-Assist with mobility and transfers, including using assistive devices when necessary
-Provide emotional support and companionship to clients
-Perform light housekeeping tasks, including meal preparation and laundry
-Escort clients to appointments or outings as needed
-Light secretarial work: Write letters, organize mail, assist with scheduling appointments
-Assist with pet care as needed
Requirements:
-Previous experience in caregiving or a related field is required (Minimum: 1 year)
-Knowledge of patient care techniques and procedures
-Familiarity and experience with in-home care, nursing homes, hospitals, or facilities
-Valid and current Driver’s License
-Clean background check and driving record
-High School Education
-Excellent communication skills and the ability to interact effectively with clients and their families
-Compassionate and patient demeanor when working with clients
-Ability to follow instructions and work independently with minimal supervision
-Able to pass drug/alcohol screening

We provide a comprehensive ongoing needs assessment by a Registered Nurse identifying an individual’s and his/her support system’s strengths, weaknesses, limitations, objectives and service needs. With the focus on health, psychosocial and environmental factors we develop an individualized plan of care based on the needs assessed. The plan of care is tailored to fit changing needs and thoroughly reviewed with the client, family members and/or responsible parties.
