By living a healthy lifestyle, you can lower your risk for heart disease.
- Choose healthy food and drinks. Prepare meals that are low in sodium and unhealthy fats.
- Get regular physical exercise. A brisk 30-minute walk daily would help decrease your risk for heart disease.
- Take charge of your medical conditions.
- Take your medications as prescribed and keep your appointments with your doctor.
- Manage your blood pressure.
- Speak to your doctor about managing your cholesterol levels.
- Manage your stress. Try deep breathing exercises or meditation.
- Ensure you get at least 7-8 hours of sleep a night.

We offer a wide range of services to meet a variety of needs. We provide a comprehensive ongoing needs assessment by a Registered Nurse identifying an individual’s and his/her support system’s strengths, weaknesses, limitations, objectives and service needs. With the focus on health, psychosocial and environmental factors we develop an individualized plan of care based on the needs assessed. The plan of care is tailored to fit changing needs and thoroughly reviewed with the client, family members and/or responsible parties.
